Course evaluation form

For teachers, trainers and course coordinators who want honest feedback at the end of a course or at the halfway point. Students don't sign in and aren't asked who they are. You get the same scale for every course, so results can be compared.

What this form asks, and why

  • Course name or coderequired One sheet can hold every course; filter by this column.
  • Instructor For team-taught courses; leave it out if there is one teacher.
  • The course goals were clearrequired A statement to agree or disagree with is quicker to answer than a 1–10 score.
  • The materials helped me learnrequired Separates 'the content was poor' from 'the teaching was poor'.
  • The pace was Two ends and a middle, so 'too fast' and 'too slow' don't average out to 'fine'.
  • Compared with similar courses, the workload was Relative to other courses, because students judge workload by comparison.
  • Overall, how would you rate this course?required The one rating to track from term to term: count the share of 'Excellent' and 'Good' per course.
  • What should stay the same? Stops a good part of the course being cut by accident.
  • What one change would most improve this course? Asking for one change gets a priority, not a list.

Fields people often add

  • Expected grade (to read ratings in context)
  • Hours per week spent on the course
  • Would you recommend this course?

Common mistakes

  • Promising anonymity you can't keep: there are no name fields, but in a class of six, a timestamp or a distinctive comment can still point to one student. Say so, and read small classes in aggregate.
  • Reading results before grades are final: students hold back if they think a teacher could react.
  • Running it only at the end: a short halfway version gives the current class the benefit.
  • Editing the scale between terms: answers stop lining up. The _form_version column shows which version each row came from.
