Anonymous feedback form
For managers, HR, schools and community groups who want honest feedback people won't put their name to. The form asks nothing about who is answering. This page also shows what can still identify someone, which matters more in a team of eight than in a company of 800.

What this form asks, and why
- What is it about? Groups feedback without asking for anyone's team or role.
- Your feedbackrequired The only required question, so nobody is forced into detail.
- What would you change? Turns a complaint into something you can act on.
- Have you raised this before? Shows when the usual channels aren't working, without saying who.
Fields people often add
- A rating of how things are going (1–5)
- Department (only in organisations large enough that it doesn't identify people)
Common mistakes
- Forgetting the timestamp: the sheet has no name, email or IP address, but _submitted_at is exact. In a small team, 'sent at 9:02, just after the meeting' can point to one person. Send the link to everyone at once and read responses in a batch.
- Adding 'harmless' questions: team plus role plus years of service can narrow down to one person. Every extra demographic question makes the form less anonymous.
- Making more questions required: someone who can't answer without revealing themselves will give up instead.
- Using it for serious reports such as harassment or safety: you can't reply to an anonymous sender. Point people to a proper reporting channel too.
