IT request form

For small IT teams, office managers and the one person everyone asks about the printer. Requests arrive in one queue instead of chats and hallway conversations. Two plain questions show what to fix first.

What this form asks, and why

  • Your namerequired Who to follow up with.
  • Work emailrequired Where you reply; the form doesn't send updates itself.
  • What is it about?required Routes access requests and hardware faults to different people.
  • Who is affected?required One half of priority: an outage for everyone beats one slow laptop.
  • Can you work right now?required The other half. Asking 'can you work?' gets an honest answer where 'priority?' gets 'high'.
  • Short summaryrequired The line you scan in the sheet; short so it fits the column.
  • What happened, and what have you tried?required 'What have you tried' saves asking 'have you restarted it?'
  • Exact error message The exact wording is searchable; a paraphrase isn't.
  • Device name or asset tag Identifies the machine without a site visit.
  • Needed by (for requests) Turns 'ASAP' for a new starter's laptop into a real date.

Fields people often add

  • Location or desk
  • Phone or extension
  • Manager approval needed (yes/no)

Common mistakes

  • One 'priority' dropdown: almost everyone picks 'high'. Who is affected plus whether they can work gives you priority without asking for it.
  • Letting people send passwords: say on the form that you'll never ask for one, so a typed password doesn't end up in a shared sheet.
  • Asking for screenshots: this form doesn't take attachments yet, so ask for the exact error text instead.
  • Questions that only fit one category: there is no conditional logic yet, so everyone sees every question. Keep questions that fit all requests, or publish a separate access-request form.
