Work order form
For facilities teams, maintenance supervisors and small trade businesses turning requests into assigned jobs. The person dispatching fills it in; the sheet becomes the running log you filter by technician, priority or due date. (Reporting a problem? Use the maintenance request form.)

What this form asks, and why
- Original request reference Links the job back to the request it came from.
- Requested by Who to tell when the job is done.
- Location or assetrequired A room number or asset tag, so the technician doesn't have to ask.
- Traderequired Lets you batch jobs by trade.
- Work to be donerequired What 'done' means for this job, written before work starts.
- Priorityrequired Each priority states its response time, so 'high' means the same to everyone.
- Assigned torequired A list instead of free text, so one person isn't spelled three ways. Replace the options with your team.
- Due byrequired Sort ascending for the earliest due first; filter out finished jobs to see what's overdue.
- Estimated hours A number you can total per technician to see who is overloaded.
- Parts and materials needed Ordered before the visit, not discovered during it.
Fields people often add
- Estimated cost
- Downtime allowed (yes/no)
- Safety notes or permits needed
Common mistakes
- Expecting the form to track progress: it creates the job. Add 'Status' and 'Completed on' columns in the sheet; columns you add are kept when the form changes.
- Waiting for sequential numbers: each row gets a unique _submission_id, not WO-0001. If you need running numbers, add your own column.
- Free-text assignees: 'Sam', 'sam' and 'Sam K' become three people when you filter.
- Adding up costs inside the form: it can't. Record hours and parts here, and price them on a separate tab.
