Job application form

For cafés, shops, clinics, nonprofits and small firms hiring without an applicant tracking system. Every applicant becomes a row you can filter by role and availability. It leaves out date of birth, family and health questions, which can be used as evidence of discrimination.

What this form asks, and why

  • Phonerequired Hourly roles are often filled with a quick call.
  • Positionrequired One form for every opening; filter by this column per role.

Fields people often add

  • Cover letter
  • Driver's licence (for driving roles)
  • Salary expectations
  • How did you hear about this role?

Common mistakes

  • Asking for date of birth, marital status or number of children: in the US, the EEOC warns these questions may be used as evidence of intent to discriminate. If a role has a minimum age, ask 'Are you 18 or over?'
  • Asking about health or disability before an offer: US law (the ADA) generally prohibits it at the application stage.
  • Collecting Social Security or tax numbers on the application: you only need them once someone is hired, through payroll.
  • Requiring a CV upload: this form can't take files yet, and many hourly applicants don't have a CV. A link field plus an experience box gets more applications.
