Registration form
For classes, courses, sports clubs and holiday programmes: anything people join for a term or a season rather than a single day. The sheet becomes your roster, with the session, the age group and the emergency contact on every row.
Copies the form into your account after you sign in with Google. You can change anything before publishing.

What this form asks, and why
- Participant's full namerequired The person attending, which is not always the person filling in the form.
- Age grouprequired Enough to place people in the right group without collecting a date of birth.
- Sessionrequired One column to count per session, so you can see when a slot is full.
- Experience Helps the instructor split a mixed group on the first day.
- Parent or guardian name (if under 18) Optional, so adults can skip it; for children it records who signed them up.
- Contact emailrequired Where the confirmation and any timetable changes go.
- Contact phonerequired For same-day messages such as a cancelled session.
- Emergency contact (name and phone)required A second person to call. It's on the same row as the session, so it's easy to find in a hurry.
- Anything the instructor should know? A private place for needs the instructor should plan for.
- I agree to the programme terms and to being contacted about this registration.required A TRUE/FALSE record that the terms were accepted.
- I agree that photos including the participant may be used on our website and social media. Separate and optional, so a family can join without agreeing to photos. FALSE means no photos.
Fields people often add
- School or club the participant belongs to
- Membership or player number
- How did you hear about us?
- Second emergency contact
Common mistakes
- Expecting the form to stop at capacity: it doesn't. Count rows per session on a second tab with COUNTIF, take places in _submitted_at order, and remove a full session from the form before republishing.
- Bundling photo permission into the required terms: families who say no to photos can't register at all.
- Asking for a full date of birth when an age group is all you use: it's more personal data to keep safe.
- One form for a whole family: rows stop matching people. Ask for one registration per participant.
