What this form asks, and why

  • Full namerequired For rosters and name badges.
  • Emailrequired For the welcome and rostering emails.
  • Mobilerequired Volunteer coordination happens by text on the day.
  • Are you 18 or over?required Under-18s often need a parent's permission and some roles aren't open to them.
  • Which roles interest you?required Several can apply. They're written as one cell of labels, so a filter for 'Driving' finds every driver.
  • When are you usually free?required Fixed choices you can count: COUNTIF(column, "*Weekends*") tells you how many people are free at weekends.
  • Skills or experience you'd like to use Where you find the bookkeeper or photographer you didn't know you had.
  • Working with children check number, if you have one Optional, and only a record: the check itself is done on the issuing agency's own service.
  • Check expiry date Its own date column, so sorting it shows whose check runs out first.
  • Emergency contact (name and phone)required Needed before anyone works a shift.
  • I agree that you may keep these details to organise volunteering.required Records that the volunteer agreed you could keep their details.

Fields people often add

  • T-shirt size
  • Languages spoken
  • Driver's licence held (yes/no)
  • Parent or guardian contact for under-18s

Common mistakes

  • Treating a typed check number as a cleared check: it's only a claim until you verify it with the issuer, such as Service NSW for a Working With Children Check (free for volunteers in NSW).
  • Free-text availability ('most weekends, not school holidays'): you can't filter or count it. Use fixed choices and leave details for the skills box.
  • Forgetting under-18s: without the age question you find out at the first shift.
  • Asking every volunteer for a check: rules have exemptions. In NSW and Victoria, for example, under-18s don't need one, and parents volunteering in their own child's activity often don't either.
